Dr Shazid Karim
Dr Karim has been a GP since 2011 and a partner at Cobham Health Centre since 2014. In addition to his role as a Non-Executive Director for GPHP Health Partners Ltd, he acts as a Community Lead GP for Surrey Downs Health and Care (SDHC) allowing him to bring an overarching insight to the local healthcare landscape.
He holds a PGC in Healthcare Management and Leadership providing him with the skills to ensure all the projects he supports continue to progress in ever-changing times. His personable nature coupled with his critical evaluation skills allow him to provide strategic direction to his varied roles. Hence he is well placed to lead Cobham & Oxshott Primary Care Network (PCN) to ensure that the PCN serves its patients through the provision of high-quality healthcare, accessed locally.
Oxshott Medical Practice GP Partner & PCN Representative
Dr Mark Jenkins
Dr Jenkins has been a GP partner since 2003 and moved to Oxshott Medical Practice in 2012. He is also the Medical Director for Epsomedical, based out of Epsom and Cobham Day Surgery Units giving him a unique perspective of secondary care from his background in primary care. He is a Fellow of the Royal College of General Practitioners and a trustee of a charity, the Surrey Medical Benevolent Society.
Pharmacist
Ami Gore
Ami Gore has been a working as a Clinical Pharmacist with Cobham and Oxshott Practices since January 2019. She has been a qualified Pharmacist since 1992 and started her career in Hospital Pharmacy and then moved into Community Pharmacy for many years prior to this current role. She is a non-medical prescriber with her field of interest being the management of Chronic Asthma . She undertakes medication reviews, answers prescription queries and undertakes work to ensure the Practices prescribe medication safely and appropriately.
First contact practitioner – Paramedic
Eleanor Berry
Eleanor Berry (Ellie) began her career in the ambulance service in 2015. Working for front line ambulance services South East coast and later London Ambulance Service as a paramedic. By the end of her front line career, she was qualified to train at levels PPED1 and PPED2 and was a well-established member of the team and she still frequently completes bank shifts for London Ambulance Service and works in triage for West Middlesex Accident and Emergency.
Migrating from critical to primary care in January 2023, Ellie then began training as a first contact practitioner (FCP), completing her FCP portfolio and primary care training in the first 9 months of her role.
Eleanor has recently been accepted on to the Advanced Clinical Practice master’s degree and hopes that this will further her understanding of primary care and aid her clinical assessment, expanding her skillset. Her main focus is improving patient quality of life in a holistic way aiming to ensure that patients get the best clinical care but also social and emotional support.
Social Prescriber
Paul Chouls
Paul has been a Social Prescriber since October 2023, joining Cobham & Oxshott PCN in April 2024. He was previously a caseworker for Independent Age, working with Older Adults who identified as lonely and isolated and prior to that for Surrey Borders NHS Trust in work services, supporting people recovering from mental health issues; preparing them to return to paid employment, volunteering or training/education.
Paul has a personable, approachable manner with a wealth of knowledge on local groups and services, and is able to link individuals to those services that will support health and well-being.
